Enrollment Process

Step 1: Contact PACE

When you call or come to our center, we will explain our program and services; eligibility requirements; and payment options for PACE services. We will ask some questions about the challenges you or your loved one face and help you determine if PACE is the right fit for you or your loved one. We will then schedule an in-home visit with an enrollment specialist.

Step 2: Home Visit

Our Intake Coordinator(s) will meet with you and your loved one in your home to further discuss your needs. We will complete some assessments and apply for Medicaid for you, if needed. We will then schedule additional assessments with other members of our care team such as physicians, therapists, and others.

Step 3: Team Discussion

Next, the interdisciplinary team--made up of the healthcare professionals at PACE--share their assessments with each other and create a care plan addressing your needs. Your care plan is designed just for you and is optimally crafted to meet your personal needs and goals.

Step 4: Enrollment Visit

Once you have been approved for PACE, your enrollment specialist will make an appointment to come back to your home and review your care plan with you. They will then walk you through signing the enrollment papers. Once you are enrolled, PACE services will start on the first of the next month.
