Program Eligibility

PACE of Southwest Michigan provides seniors in our community the care they need
to remain living safely at home. We serve seniors from all walks of life. Our program can enroll individuals who meet the following eligibility requirements:

1. Participants must be 55 years of age or older
2. Able to safely live in the community with the help of PACE
3. Live in Berrien, Cass, or Van Buren County
4. Qualify for nursing home level of care

Nursing Home Level of Care

In order to be clinically eligible for enrollment in PACE, you must be struggling with health issues that may jeopardize your independence. There are many illnesses and conditions which may qualify you for PACE. Our enrollment specialists can help determine your eligibility.

 

PACE Service Area

PACE of Southwest Michigan serves those who live in Berrien, Cass, or Van Buren Counties including the cities of Benton Harbor, Niles, South Haven, Paw Paw, and more.
