Coordinated Medical Care

Our team of Physicians, Nurses, and Pharmacists monitor and address current medical conditions, changes in health or behavior, and plan for the long term healthcare needs of our participants. We provide primary care, all prescription medications, coordinate care with specialists, and provide onsite services.

PACE provides all primary medical care and coordinates with specialists to ensure you are following the correct treatment plan and taking the right medications.

Medical Specialists

PACE coordinates medical care with Cardiology, Nephrology, Gastroenterology, ENT, Orthopedic and other Specialists in our community. Each participant has a treatment plan that identifies needed services, testing, medications and follow up appointments.

Prescription Medications

Our pharmacist works on site a few days per week to coordinate all primary and specialty medications.  From easy to access blister packs to tamper resistant automatic pill dispensers, prescriptions are packaged to fit the needs of our participants and caregivers.  Medication changes or refills are delivered to the home or are sent home with our participants after visiting the day center.
